To qualify for a free tablet through Lifeline, you may need to provide proof of eligibility. If we can’t verify your information automatically, you’ll be asked to upload official documents. What You May Need to Show?
1. Program Participation:
If you’re enrolled in a qualifying program (like SNAP or Medicaid), submit a document that includes:
- Your (or dependent’s) full name
- Program name
- Issuing agency
- A date within the past year or with a future expiration
Accepted Documents:
Benefit letter, approval notice, or verification of benefits.
2. Household Income
For income-based eligibility, submit proof showing annual income and your full name. If using pay stubs, they must cover 3 consecutive months within the last 12 months.
Accepted Documents: Tax return, paycheck stub, unemployment benefits, child support order.
3. Identity & Date of Birth
Your documents must include your full name and date of birth.
Accepted Documents:
Government ID, birth certificate, Social Security card, green card.
4. Social Security Number or Tribal ID
Provide a document showing your full name and the last 4 digits of your SSN or full Tribal ID.
Accepted Documents:
SSN card, W-2, tax return, government-issued ID.
5. Address
You must verify your physical address (not a P.O. Box).
Accepted Documents:
Utility bill, lease or mortgage statement, driver’s license, tax return.
6. Duplicate Address
If someone at your address already gets Lifeline, complete the Household Worksheet to confirm your household qualifies separately.
7. Proof of Life
You must confirm you’re alive and still qualify.
Accepted Documents (dated within 3 months):
Recent paystub, utility bill, benefits letter.
8. Emancipated Minor
If under 18, you must provide a court-issued document proving emancipation.
